Despite having more tools than ever, fewer than 1 in 5 Canadians with type 2 diabetes reach their glycemic, blood pressure, and lipid targets. Continuous glucose monitoring (CGM) offers pharmacists a practical way to close that gap. This interactive session moves quickly from CGM fundamentals to real pharmacy decision making. Participants will review how to read time in range data and the ambulatory glucose profile using a simple four-step approach, then apply it to three cases: a newly diagnosed patient resistant to medication, a patient with hidden hypoglycemia on a sulfonylurea, and an older adult living alone on basal insulin. The session closes with a five-step workflow for identifying candidates, starting the conversation, and following up within existing pharmacy touchpoints.
